After almost 10 years as my primary care physician, Dr. Thomas Firth left the Portland area and didn't even leave a forw...arding number or send a letter notifying me that is was moving his practice to Corvallis, OR. Not only do I have to find a new PCP, I don’t know how do go about getting my medical records – VERY UNPROFESSIONAL! more details
