Started as a new patient. Asked for one of my Rx without refills to be issued as 90 day Rx to my insurance's mail order ...supplier. Instead of the $10 generic I requested a $125 patented 30 day was ordered. When I requested a second time that the correct Rx be placed, instead the same $125 patented 30 day was ordered. more details
